## Problem

In the document detail view (e.g., TRCKR-400 QA Report), several form fields are rendering as white boxes that are not visibly displaying their content against the dark background.

## Affected Fields

The screenshot shows these fields have poor contrast:
- **Title** field - white input box with white text (invisible)
- **Type** field - white background making text hard to read
- **Entity ID** field - white background, invisible content
- **Tags** field - white background

## Current Behavior

- Input fields render with white backgrounds
- Text color appears to be white or very light
- Content is functionally present but visually invisible
- User cannot read or edit field values effectively

## Expected Behavior

- All input fields should have proper contrast against the dark theme
- Text should be readable (light text on dark backgrounds)
- Input backgrounds should match the dark theme styling
- Should match the styling of the EasyMDE editor area below (which has proper dark theme)
