When editing an issue with no relationships, the relationships section should show 'None' with a plus button to add one, rather than displaying empty fields or complex UI.

## Current Behavior
The relationships section in the issue edit modal likely shows empty form fields or a confusing layout when there are no existing relationships.

## Desired Behavior
When an issue has no relationships:
- Display 'None' text
- Show a '+' button next to it
- Clicking '+' opens the existing relationship add form/modal

When an issue has relationships:
- Display them as normal (with ability to remove)
- Still show '+' button to add more

## Implementation

1. **Template changes** (`server/templates/partials/issue_edit_modal.html`):
   - Check if relationships list is empty
   - If empty: show 'None' + add button
   - If not empty: show relationship list + add button
   - Add button triggers existing relationship add form

2. **Behavior**:
   - '+' button opens relationship add modal/form
   - Reuse existing relationship creation UI
   - After adding, refresh the relationships section

Implemented simplified relationships display: - Empty relationships now show 'None' with a '+' button - Clicking '+' reveals a form to add relationships - When relationships exist, they are displayed with remove buttons and a '+' button to add more - Form can be cancelled to return to the simplified view - All tests pass
